Struggling with the look of your nose but only want to make necessary changes and not remove too much structure or tissue? A preservation rhinoplasty may be just the solution you’ve been looking for. With the expertise of dual-board-certified plastic surgeon Dr. David Gilpin, you can achieve your aesthetic goals once and for all.

What is Preservation Rhinoplasty?

Preservation rhinoplasty (also called dorsal preservation rhinoplasty) is a unique type of rhinoplasty and facial plastic surgery technique that is designed to alter the shape of the nose while also maintaining as much of your natural bone, tissue, and cartilage as possible. With this procedure, you can achieve a more natural-looking result and reduce the risk of complications that can often come with a traditional rhinoplasty.

By removing small amounts of bone and cartilage from underneath the nasal bridge, you can achieve the results you’re looking for without the removal of large amounts of bone and cartilage. It can treat a nasal hump, nasal tip concerns, and address a number of issues with the bridge of the nose. It can also treat a deviated septum or a crooked nose.

With this technique, you can expect an easier recovery process, long-term outcomes, a unique look that’s specifically yours, and no need for grafts.

Ideal Candidates for Preservation Rhinoplasty Surgery

Most patients who struggle with the shape or size of their noses but are also looking to maintain as much bone and tissue as possible are typically ideal candidates for this procedure. That said, there are also some criteria we follow to determine if a preservation rhinoplasty is right for a specific patient.

You should be in good overall health, have realistic expectations of treatment, be a nonsmoker, and want to correct a nasal bump or reduce the projection of your nose.  We also recommend that you not be pregnant or breastfeeding.

Preservation Rhinoplasty Pre Op Recommendations

Before your preservation rhinoplasty, you’ll want to schedule a consultation with Dr. Gilpin. He will walk you through the treatment process, review your medical history, and discuss your goals. Once he has all your information, he’ll develop a custom treatment plan that ensures all your needs are met.  He will also give you a list of guidelines to follow to make sure you’re ready for surgery.

In the weeks before surgery, you will want to stop taking certain medications like blood thinners, avoid alcohol intake, and stay away from excess sun exposure. You will also want to arrange with a friend or family member to drive you to and from surgery. Picking up any prescriptions and building a comfortable space in your home is also recommended.

Preservation Rhinoplasty Procedure

To start your preservation rhinoplasty surgery, we’ll begin by administering general anesthesia for your comfort. From there, Dr. Gilpin will make the necessary incisions to access the underlying structures of the nose. He will then remove the necessary bones and tissues while still keeping your primary nasal structure and reshape your nose to your specifications.

Once everything is removed and reshaped, incisions will be closed, and you’ll be able to start the recovery process. The whole procedure takes between one to two hours.

Results and Recovery for Preservation Rhinoplasty

After your surgery, you will likely have some swelling, bruising, and discomfort. Dr. Gilpin will prescribe pain medication to ensure your recovery is comfortable. He will also give you detailed instructions on what to avoid for proper healing.

To start your recovery, you should rest as much as possible, drink plenty of water, and avoid strenuous activities. You will also want to take 1-2 weeks off work, although it can depend on the type of job you have.

As for results, you may see some initial improvements in the first few days, but the swelling and bruising will hide most of the changes. In the weeks and months following treatment, the swelling and bruising will fade, and your full results will be visible.

FREQUENTLY ASKED QUESTIONS

Is nose job permanent?

A well-performed rhinoplasty by a facial plastic surgeon is expected to be permanent. However, the younger you are when having rhinoplasty, there is a small but increased chance of your nose changing shape with age.

CAN I WEAR GLASSES AFTER MY NOSE SURGERY?

Patients who wear contact lenses can resume wearing contacts immediately after nose surgery; however, patients who wear glasses will need to take extra precaution. If you need to wear glasses during the first four-to-six weeks after nose surgery, it's recommended that you either tape your glasses to your forehead or buy a special device that allows the glasses to rest on your cheeks instead of your nose. These devices are commonly used by patients after rhinoplasty.

Why do nose jobs collapse?

Rhinoplasties collapse when they are performed in such a way that eliminates the underlying structure of nasal bone and supportive tissue. Over time, the skin above this structure sags and changes the appearance of the nose.

WHEN WILL THE STITCHES BE REMOVED AFTER RHINOPLASTY?

Stitches are typically removed six or seven days after nose surgery.

WHEN CAN I EXPECT TO SEE RESULTS FROM NOSE SURGERY?

You will probably notice an immediate improvement in the appearance of your nose, but the final results will become more evident as swelling goes down over the coming weeks and months after your procedure. It may take up to a year for the final results of your rhinoplasty to become apparent. It's normal to feel a bit anxious to see the full results, but keep in mind that your body needs time to fully heal. Every day your swelling will subside, allowing you to adjust to your newly sculpted nose.

WHERE WILL MY RHINOPLASTY IN NASHVILLE BE PERFORMED?

Dr. Gilpin's procedures are typically performed at the Cool Springs Surgery Center, which is a fully accredited, state-of-the-art surgical center conveniently located in Nashville, TN.

WILL I BE ABLE TO GO HOME AFTER MY NOSE SURGERY IS COMPLETE?

Yes, after your nose surgery is complete you will be able to go home. Rhinoplasty is usually performed on an outpatient basis. As a caveat, you will still be under the effects of anesthesia after surgery, so you will need a responsible adult to drive you home afterwards. Also, you might want to consider arranging for someone to help you out for a day or two after surgery. This will allow you to relax and maximize your recovery time from rhinoplasty.

DOES RHINOPLASTY LEAVE NOTICEABLE SCARS?

Your rhinoplasty scars will be virtually undetectable. The incision is made across the columella, which is the bridge of skin and tissue between the nostrils. The remainder of the incisions are all made inside of the nose.u00a0At the conclusion of the nose surgery, Dr. Gilpin meticulously closes the columellar incision with fine sutures, which leaves almost no trace of the incision.

WHAT ARE THE POSSIBLE COMPLICATIONS FROM RHINOPLASTY?

Complications from rhinoplasty are rare, especially when performed by a board-certified surgeon on appropriately selected candidates. However, as with any surgery, risks are still possible. General complications after rhinoplasty include adverse reaction to anesthesia, poor healing, nosebleed, and infection.

HOW LONG DOES IT TAKE TO RECOVER FROM RHINOPLASTY? WILL I BE IN PAIN AFTERWARDS?

Most of Dr. Gilpin's Nashville nose surgery patients report minimal pain after the procedure, and the most common concern is nasal congestion and “stuffiness” because of post-operative swelling and bruising. Patients will need about one week of downtime to recover from rhinoplasty surgery. During this time, it's important to be very gentle with your nose when washing your face or applying cosmetics. After the first five-to-seven days, people can usually return to work and resume their regular daily activities. However, strenuous exercise should be avoided until Dr. Gilpin determines that your nose has healed properly. Dr. Gilpin's follow-up schedule includes an appointment four days after surgery, then one week, two weeks, six months, and one year. At your first follow-up appointments, he can make sure you're healing optimally and clear you for physical activity, when appropriate. In the case of non-surgical rhinoplasty, downtime is very minimal. Patients might experience minor swelling or bruising, but this typically doesn't last more than a day and can usually be covered by makeup.

Is it better to go to an ENT or plastic surgeon for rhinoplasty?

It depends on your goals. An ENT surgeon may be ideal if your primary concern is functional issues like breathing, while a plastic surgeon focuses on the cosmetic aspects of reshaping the nose. Many patients prefer a specialist with dual expertise in ENT and facial plastic surgery to address function and aesthetics.

Where is the best place to get a nose job?

The best place to get a rhinoplasty is with a board-certified plastic surgeon or facial plastic surgeon experienced in rhinoplasty. Look for a provider with extensive before-and-after photos, positive reviews, and expertise in rhinoplasty surgery.
