The Closed Rhinoplasty Procedure
Closed rhinoplasty is typically performed under general anesthesia and takes about one to two hours, depending on the complexity of the case. Unlike open rhinoplasty, which involves an external incision across the columella (the strip of skin between the nostrils), closed rhinoplasty is performed entirely through incisions hidden inside the nostrils. These internal incisions allow board-certified rhinoplasty surgeon, Dr. Gilpin, to carefully access the nasal framework, specifically the bone and cartilage, without lifting the nasal skin.
Once inside, Dr. Gilpin uses specialized instruments to reshape the nose based on your treatment plan. If there’s a dorsal hump (a bump on the bridge of the nose), it may be gently shaved or reduced. If the tip needs refinement, he can sculpt or reposition the lower lateral cartilages to create a more defined or lifted appearance. For patients who need better symmetry or improved breathing, the nasal septum or internal valves may also be addressed during the same procedure.
Because there is no external opening, the skin drapes more naturally over the modified structures, often resulting in a smoother, more organic-looking outcome. This internal approach also preserves the structural integrity of the nose, which is particularly beneficial for long-term support and natural movement.
The entire procedure generally takes between one and two hours, depending on the complexity of your case. After surgery, you’ll be moved to a comfortable recovery area where our team will monitor you as you wake up from anesthesia.
