At Gilpin Facial Plastics + Aesthetics, we believe that enhancing your natural beauty should be a seamless and rewarding experience. If you’re considering rhinoplasty and are looking for a refined, less invasive option, closed rhinoplasty may be the right approach for you. Led by double board-certified facial plastic surgeon Dr. David Gilpin, our Nashville team is proud to offer this advanced technique to help patients achieve their aesthetic goals with precision and minimal downtime.

What is Closed Rhinoplasty?

Closed rhinoplasty, also called a closed nose job, is a surgical technique used to reshape the nose without making any external incisions. Unlike an open rhinoplasty, all surgical access is gained through small incisions inside the nostrils, which means there are no visible scars left behind. This method allows Dr. Gilpin to adjust the bone and cartilage with great care while preserving the natural structures and support of the nose. Closed rhinoplasty is often chosen for patients who require subtle to moderate changes in nasal shape or structure, particularly involving the nasal bridge and tip.

Ideal Candidates for Closed Rhinoplasty

The ideal candidate for closed rhinoplasty surgery is someone who is in overall good health, does not smoke, and has realistic expectations about what the procedure can achieve. If you’re looking to refine the appearance of your nose, perhaps by smoothing a dorsal hump, correcting asymmetry, or subtly lifting the nasal tip without significant structural changes or a history of prior nasal surgery, you may be an excellent candidate. During your consultation, Dr. Gilpin will assess your anatomy and aesthetic goals to determine if this approach is the most appropriate fit for you.

The Closed Rhinoplasty Procedure

Closed rhinoplasty is typically performed under general anesthesia and takes about one to two hours, depending on the complexity of the case. Unlike open rhinoplasty, which involves an external incision across the columella (the strip of skin between the nostrils), closed rhinoplasty is performed entirely through incisions hidden inside the nostrils. These internal incisions allow board-certified rhinoplasty surgeon, Dr. Gilpin, to carefully access the nasal framework, specifically the bone and cartilage, without lifting the nasal skin.

Once inside, Dr. Gilpin uses specialized instruments to reshape the nose based on your treatment plan. If there’s a dorsal hump (a bump on the bridge of the nose), it may be gently shaved or reduced. If the tip needs refinement, he can sculpt or reposition the lower lateral cartilages to create a more defined or lifted appearance. For patients who need better symmetry or improved breathing, the nasal septum or internal valves may also be addressed during the same procedure.

Because there is no external opening, the skin drapes more naturally over the modified structures, often resulting in a smoother, more organic-looking outcome. This internal approach also preserves the structural integrity of the nose, which is particularly beneficial for long-term support and natural movement.

The entire procedure generally takes between one and two hours, depending on the complexity of your case. After surgery, you’ll be moved to a comfortable recovery area where our team will monitor you as you wake up from anesthesia.

Results and Recovery for Closed Rhinoplasty

One of the advantages of closed rhinoplasty is the streamlined recovery process. Most patients experience bruising and swelling around the eyes and nose during the first week, which gradually improves over time. You’ll likely wear a small splint for about a week, but there are no visible stitches to remove since all incisions are inside the nose.

While you’ll start to see changes once the swelling subsides, it can take several months for the final, refined results to fully emerge as the tissues settle and heal. Dr. Gilpin will monitor your progress closely and provide personalized care during each follow-up visit to ensure your recovery is smooth and successful.

FREQUENTLY ASKED QUESTIONS

Is nose job permanent?

A well-performed rhinoplasty by a facial plastic surgeon is expected to be permanent. However, the younger you are when having rhinoplasty, there is a small but increased chance of your nose changing shape with age.

Why do nose jobs collapse?

Rhinoplasties collapse when they are performed in such a way that eliminates the underlying structure of nasal bone and supportive tissue. Over time, the skin above this structure sags and changes the appearance of the nose.

HOW LONG DOES IT TAKE TO RECOVER FROM RHINOPLASTY? WILL I BE IN PAIN AFTERWARDS?

Most of Dr. Gilpin's Nashville nose surgery patients report minimal pain after the procedure, and the most common concern is nasal congestion and “stuffiness” because of post-operative swelling and bruising. Patients will need about one week of downtime to recover from rhinoplasty surgery. During this time, it's important to be very gentle with your nose when washing your face or applying cosmetics. After the first five-to-seven days, people can usually return to work and resume their regular daily activities. However, strenuous exercise should be avoided until Dr. Gilpin determines that your nose has healed properly. Dr. Gilpin's follow-up schedule includes an appointment four days after surgery, then one week, two weeks, six months, and one year. At your first follow-up appointments, he can make sure you're healing optimally and clear you for physical activity, when appropriate. In the case of non-surgical rhinoplasty, downtime is very minimal. Patients might experience minor swelling or bruising, but this typically doesn't last more than a day and can usually be covered by makeup.

WHERE WILL MY RHINOPLASTY IN NASHVILLE BE PERFORMED?

Dr. Gilpin's procedures are typically performed at the Cool Springs Surgery Center, which is a fully accredited, state-of-the-art surgical center conveniently located in Nashville, TN.

CAN I WEAR GLASSES AFTER MY NOSE SURGERY?

Patients who wear contact lenses can resume wearing contacts immediately after nose surgery; however, patients who wear glasses will need to take extra precaution. If you need to wear glasses during the first four-to-six weeks after nose surgery, it's recommended that you either tape your glasses to your forehead or buy a special device that allows the glasses to rest on your cheeks instead of your nose. These devices are commonly used by patients after rhinoplasty.

WILL I BE ABLE TO GO HOME AFTER MY NOSE SURGERY IS COMPLETE?

Yes, after your nose surgery is complete you will be able to go home. Rhinoplasty is usually performed on an outpatient basis. As a caveat, you will still be under the effects of anesthesia after surgery, so you will need a responsible adult to drive you home afterwards. Also, you might want to consider arranging for someone to help you out for a day or two after surgery. This will allow you to relax and maximize your recovery time from rhinoplasty.

DOES RHINOPLASTY LEAVE NOTICEABLE SCARS?

Your rhinoplasty scars will be virtually undetectable. The incision is made across the columella, which is the bridge of skin and tissue between the nostrils. The remainder of the incisions are all made inside of the nose.u00a0At the conclusion of the nose surgery, Dr. Gilpin meticulously closes the columellar incision with fine sutures, which leaves almost no trace of the incision.

WHEN WILL THE STITCHES BE REMOVED AFTER RHINOPLASTY?

Stitches are typically removed six or seven days after nose surgery.

WHAT ARE THE POSSIBLE COMPLICATIONS FROM RHINOPLASTY?

Complications from rhinoplasty are rare, especially when performed by a board-certified surgeon on appropriately selected candidates. However, as with any surgery, risks are still possible. General complications after rhinoplasty include adverse reaction to anesthesia, poor healing, nosebleed, and infection.

WHEN CAN I EXPECT TO SEE RESULTS FROM NOSE SURGERY?

You will probably notice an immediate improvement in the appearance of your nose, but the final results will become more evident as swelling goes down over the coming weeks and months after your procedure. It may take up to a year for the final results of your rhinoplasty to become apparent. It's normal to feel a bit anxious to see the full results, but keep in mind that your body needs time to fully heal. Every day your swelling will subside, allowing you to adjust to your newly sculpted nose.

Is it better to go to an ENT or plastic surgeon for rhinoplasty?

It depends on your goals. An ENT surgeon may be ideal if your primary concern is functional issues like breathing, while a plastic surgeon focuses on the cosmetic aspects of reshaping the nose. Many patients prefer a specialist with dual expertise in ENT and facial plastic surgery to address function and aesthetics.

Where is the best place to get a nose job?

The best place to get a rhinoplasty is with a board-certified plastic surgeon or facial plastic surgeon experienced in rhinoplasty. Look for a provider with extensive before-and-after photos, positive reviews, and expertise in rhinoplasty surgery.
