Revision Rhinoplasty

IN NASHVILLE, TN

Rhinoplasty is one of the most commonly performed plastic surgeries, but many patients unfortunately do not achieve the results they are looking for. Rhinoplasty is said to be one of the most challenging of the facial cosmetic procedures, meaning it is best left to expert plastic surgeons who understand the delicate structure of the nose. If you are unsatisfied with the appearance of your nose or have unresolved breathing issues following a rhinoplasty, Dr. Gilpin can help through a revision rhinoplasty in Nashville.

YOUR NASHVILLE REVISION RHINOPLASTY EXPERT

Especially if you have had a poor experience with rhinoplasty in the past, choosing the best surgeon for your revision rhinoplasty is crucial. Additionally, revision rhinoplasty is a more complex and technically challenging procedure than an initial rhinoplasty, so it is important to choose a highly skilled and experienced plastic surgeon.

Dr. David Gilpin is a double-board certified plastic surgeon who specializes in facial plastic surgery. He understands the subtle details that create a balanced, aesthetically pleasing nose shape and how to create a result that appears natural and suits the rest of your facial features. Using advanced surgical techniques and leaning on years of experience, Dr. Gilpin can correct issues from a previous rhinoplasty and address your concerns regarding both appearance and function.

IDEAL REVISION RHINOPLASTY CANDIDATES

Most patients who have had a previous rhinoplasty with unsatisfactory results are good candidates for a revision rhinoplasty. Before surgery, they should be generally healthy and, ideally, a nonsmoker. Revision rhinoplasty is a complex procedure, and not all issues can be corrected fully. Dr. Gilpin will examine the nose carefully during a consultation and explain what results you can reasonably expect. In some cases, a revision rhinoplasty may not be advisable. However, these cases are rare.

THE REVISION RHINOPLASTY PROCEDURE

A revision rhinoplasty is performed under general anesthesia. Incisions are made inside the nose, or they can be made at the base of the columella to allow for restructuring and reshaping of the nose. Because the problems are different for each patient, the revision rhinoplasty surgery needs to be customized to each individual’s unique problems.

Through these incisions, Dr. Gilpin will work to reshape the cartilage and bones to achieve your goal results. In some cases, cartilage grafts from the ear or donated rib cartilage are necessary to rebuild areas of the nose that were reduced too much in an initial rhinoplasty. Finally, Dr. Gilpin will work to preserve function or make changes that can improve breathing as necessary.

NONSURGICAL RHINOPLASTY

For some patients, a nonsurgical or liquid rhinoplasty can be beneficial, especially if the nose is disproportionately small or the nasal bridge has a bump, known as a dorsal hump. Using dermal fillers, the nose can be refined, resulting in a straighter nasal bridge, more prominent nasal tip, or refined overall shape. Like all filler injections, a liquid nose job is a temporary procedure, requiring patients to have maintenance injections every 6-12 months. You may consider choosing a nonsurgical rhinoplasty before a revision rhinoplasty to see if you prefer the results.

RECOVERY AFTER REVISION RHINOPLASTY

Recovery after your revision rhinoplasty is overall similar to the recovery process after an initial rhinoplasty. You can expect some bruising and swelling, which should dissipate within a few days. Some discomfort is normal but can be treated with pain medication. Typically, patients require about 1-2 weeks of downtime before they are ready to return to work, school, or other routine activities. Residual swelling can persist for about six months after your surgery, meaning that it can take some time before your final results are seen. Dr. Gilpin will explain what you can expect during a consultation and monitor your healing through follow-up appointments after your revision rhinoplasty.

FREQUENTLY ASKED QUESTIONS

HOW LONG DOES IT TAKE TO RECOVER FROM RHINOPLASTY? WILL I BE IN PAIN AFTERWARDS?

Most of Dr. Gilpin’s Nashville nose surgery patients report minimal pain after the procedure, and the most common concern is nasal congestion and “stuffiness” because of post-operative swelling and bruising. Patients will need about one week of downtime to recover from rhinoplasty surgery. During this time, it’s important to be very gentle with your nose when washing your face or applying cosmetics. After the first five-to-seven days, people can usually return to work and resume their regular daily activities. However, strenuous exercise should be avoided until Dr. Gilpin determines that your nose has healed properly. Dr. Gilpin’s follow-up schedule includes an appointment four days after surgery, then one week, two weeks, six months, and one year. At your first follow-up appointments, he can make sure you’re healing optimally and clear you for physical activity, when appropriate. In the case of non-surgical rhinoplasty, downtime is very minimal. Patients might experience minor swelling or bruising, but this typically doesn’t last more than a day and can usually be covered by makeup.

WHERE WILL MY RHINOPLASTY IN NASHVILLE BE PERFORMED?

Dr. Gilpin’s procedures are typically performed at the Cool Springs Surgery Center, which is a fully accredited, state-of-the-art surgical center conveniently located in Nashville, TN.

CAN I WEAR GLASSES AFTER MY NOSE SURGERY?

Patients who wear contact lenses can resume wearing contacts immediately after nose surgery; however, patients who wear glasses will need to take extra precaution. If you need to wear glasses during the first four-to-six weeks after nose surgery, it’s recommended that you either tape your glasses to your forehead or buy a special device that allows the glasses to rest on your cheeks instead of your nose. These devices are commonly used by patients after rhinoplasty.

WILL I BE ABLE TO GO HOME AFTER MY NOSE SURGERY IS COMPLETE?

Yes, after your nose surgery is complete you will be able to go home. Rhinoplasty is usually performed on an outpatient basis. As a caveat, you will still be under the effects of anesthesia after surgery, so you will need a responsible adult to drive you home afterwards. Also, you might want to consider arranging for someone to help you out for a day or two after surgery. This will allow you to relax and maximize your recovery time from rhinoplasty.

DOES RHINOPLASTY LEAVE NOTICEABLE SCARS?

Your rhinoplasty scars will be virtually undetectable. The incision is made across the columella, which is the bridge of skin and tissue between the nostrils. The remainder of the incisions are all made inside of the nose. At the conclusion of the nose surgery, Dr. Gilpin meticulously closes the columellar incision with fine sutures, which leaves almost no trace of the incision.

WHEN WILL THE STITCHES BE REMOVED AFTER RHINOPLASTY?

Stitches are typically removed six or seven days after nose surgery.

WHAT ARE THE POSSIBLE COMPLICATIONS FROM RHINOPLASTY?

Complications from rhinoplasty are rare, especially when performed by a board-certified surgeon on appropriately selected candidates. However, as with any surgery, risks are still possible. General complications after rhinoplasty include adverse reaction to anesthesia, poor healing, nosebleed, and infection.

WHEN CAN I EXPECT TO SEE RESULTS FROM NOSE SURGERY?

You will probably notice an immediate improvement in the appearance of your nose, but the final results will become more evident as swelling goes down over the coming weeks and months after your procedure. It may take up to a year for the final results of your rhinoplasty to become apparent. It’s normal to feel a bit anxious to see the full results, but keep in mind that your body needs time to fully heal. Every day your swelling will subside, allowing you to adjust to your newly sculpted nose.

Is nose job permanent?

A well-performed rhinoplasty by a facial plastic surgeon is expected to be permanent. However, the younger you are when having rhinoplasty, there is a small but increased chance of your nose changing shape with age.

Why do nose jobs collapse?

Rhinoplasties collapse when they are performed in such a way that eliminates the underlying structure of nasal bone and supportive tissue. Over time, the skin above this structure sags and changes the appearance of the nose.
